Folio 1: William Taylor, aged 45, seaman; sick or hurt, impeded respiration, with wheezing small cough, and difficult expectoration of frothy matter; put on sick list, 25 February 1805, Spithead. Discharged 28 February 1805 to the Royal Hospital Haslar.
Folios 1 and 4: Patrick Power, aged 28, seaman; sick or hurt, frequent stools with blood, tormina and tenesmus. Has suffered often from this complaint particularly in the last two years during which time he has been twice in one of the Royal Naval hospitals; put on sick list, 6 March 1805, Spithead. Discharged 7 April 1805 to duty in the Downs.
Folios 2 and 5: John Smith, aged 28, seaman; sick or hurt, pain over the right orbit and in the lumber region, pulse regular, habitual costiveness. He suffers but little from any real disease, he has been a constant complaint since he has been in the ship, though his appearance has indicated health; put on sick list, 7 March 1805, Spithead. Discharged 23 May 1805 to duty at sea.
Folio 2: James Osmond, aged 20, seaman; sick or hurt, pain about the umbilicus tenesmus, frequent stools with blood, pulse soft, appetite good, no thirst. This is the third attack of this complaint within the last 12 months, five of which he was in a naval hospital; put on sick list, 12 March 1805, Spithead.
Folios 3-4: Edward Gorgan, aged 25, seaman; sick or hurt, vertigo, nausea, thirst, soft pulse, tongue very white, erratic pains of the chest, of slight rigors; put on sick list, 13 March 1805, Spithead. Discharged 17 April 1805 in the Downs.
Folios 3, 5, 9 and 14: James Dunbar, aged 26, seaman; sick or hurt, chancre on the prepuce and the glands of the right inguin were swelled; put on sick list, 24 March 1805, Spithead. Discharged 17 April 1805 in the Downs. Returned from hospital on 20 June 1805. Discharged 28 June 1805 to duty at sea.
Folio 6: Frederick Elbricht, aged 30, seaman; sick or hurt, middle finger of the right hand much swollen, painful, red and great sense of heat; put on sick list, 6 April 1805, in the Downs. Discharged 17 April1805 to duty in the Downs.
Folio 6: Andrew Osborn, aged 26, seaman; sick or hurt, right hand swollen, slight tension of smart pain at the articulation of the middle finger with the meta carpal bones; put on sick list, 8 April 1805, in the Downs. Discharged 14 May1805 to duty at Spithead.
Folio 7: Robert Burns, aged 50, seaman; sick or hurt, by a heavy lurch of the ship he was thrown down and struck his left shoulder against the coamings of the main hatchway. The humerus luxated downward; put on sick list, 8 April 1805, at sea. Discharged 22 May1805 to duty at sea.
Folio 7: Thomas Hewson, aged 32, seaman; sick or hurt, frequent rigors of succeeding heat pulse frequent and contracted, smart cough, head and loins painful; put on sick list, 20 April 1805, at Spithead. Discharged 1 May 1805 to duty at Spithead.
Folio 8: William Thomson, aged 33, seaman; sick or hurt, contusion and wound near the third joint of the middle finger of the right hand, in consequence of the finger being jammed between the chimes of the casks; put on sick list, 22 April 1805, at Spithead. Discharged 7 June 1805 to duty at sea.

Folios 12 and 20: James Doyle, aged 20, seaman; sick or hurt; a small sloughy inflamed ulcer in the inner ankle of the right leg, also a sloughy ulcer of nearly two inches in length, with considerable tension and pain near the centre and on the anterior of each leg, scrophulous habit; put on sick list, 25 May 1805, at sea. Discharged 15 June 1805 to the hospital at St John's Newfoundland. Returned from the hospital on 20 June 1805, and discharged to duty 25 July 1805 at sea.
Folios 13-14: Daniel Sullivan, aged 30, seaman; sick or hurt; right testicle painful and enlarged, chord full and tender to the touch, scrotum inflamed and slightly thickened; put on sick list, 27 May 1805, at sea. Discharged 15 June 1805 to the hospital at St John’s Newfoundland. Returned from hospital 20 June 1805. Discharged 10 July 1805 to duty at sea.
