Folio 1: Blank.

Folio 2: Stephen Forrester, aged 22, seaman; disease or hurt, venereal disease. Put on sick list, 24 January 1800 at Portsmouth harbour. Discharged 24 January 1800 to hospital.

Folio 2: John O'Donald, aged 27, seaman; disease or hurt, venereal complaint. Put on sick list, 29 January 1800. Discharged 29 January 1800 to Haslar hospital.

Folio 2: John Guarde, aged 45, seaman; disease or hurt, [?]. Put on sick list, 29 January 1800. Discharged 29 January 1800 to Haslar hospital.

Folio 2: Daniel Callahan, aged 29, marine; disease or hurt, scropulous tumour occupying one whole side of the neck of very old standing. Put on sick list, 29 January 1800. Discharged 29 January 1800 to Haslar hospital.

Folios 2-6: Lieutenant Dumaresque; disease or hurt, complains of severe pain in his head, back, loins, sore throat, and nausea. Put on sick list, 30 January 1800. Discharged 15 February 1800 to duty.

Folios 2-6: John Calaghan, aged 13, boy; disease or hurt, complains of pain in his right knee. Put on sick list, 30 January 1800. Discharged 15 February 1800 to Haslar hospital.

Folios 2-6: Mark Stallard, aged 23, seaman; disease or hurt, contusion and fracture of the 6th rib of left side. Put on sick list, 1 February 1800 at Portsmouth. Discharged 15 February 1800 to duty.

Folios 3-25: John Stead, aged 20, seaman; disease or hurt, venereal complaint. Put on sick list, 3 February 1800 at Portsmouth. Discharged 22 March 1800 to duty.

Folios 3-4: Mr Smith, aged 19, midshipman; disease or hurt, complains of violent headache, nausea, inclination to vomit, pain in his loins and general uneasiness (suspected smallpox). Put on sick list, 3 February 1800 at Portsmouth. Discharged 8 February 1800 to Haslar hospital.

Folios 3-8: John Rowley, aged 22, marine; disease or hurt, bubo. Put on sick list, 3 February 1800 at Portsmouth. Discharged 21 February 1800 to Haslar.

Folios 3-5: James Dorman, aged 20, seaman; disease or hurt, inflammation and swelling of the right foot. Put on sick list, 4 February 1800 at Portsmouth. Discharged 15 February 1800 to duty.

Folios 5-6: Anthony [Rutold?], aged 35, seaman; disease or hurt, left fore arm and hand are much swelled and of a [shiny?] red appearance. He feels a sense of coldness in the arm and hand. On the back of the hand there is a small wound which was made by a rusty nail there is no discharge from it. Put on sick list, 13 February 1800 at Spithead. Discharged 15 February 1800 to hospital.

Folios 5-8: John Holmes, aged 23, seaman; disease or hurt, there is a large ulcer on the [?] and a bubo in his left groin. Put on sick list, 13 February 1800 at Spithead. Discharged 21 February 1800 to Haslar.

Folio 6: Timothy Lloyd, aged 24, seaman; disease or hurt, a month ago received a violent contusion on the left shoulder since which he has not been able to move it freely. He now complains of violent pain in his left breast with some cough. Put on sick list, 15 February 1800 at Spithead. Discharged 15 February 1800 to hospital.

Folio 7: John Hitchcock, aged 30, marine; disease or hurt, about 6pm fell from the main deck into the after cock pit. Found insensible no pulse to be felt at wrist, face pale and covered with cold sweat. Found a wound nearly 6 inches on the upper part of the left peretal bone extending from the back part in a curved direction to the os frontis. Poured some warm port wine immediately down his throat in about 10 minutes afterwards his pulse speed and [?] returned. The cranium appeared to be perfectly sound as far as it was [?]. Put on sick list, 17 February 1800 at Spithead. Discharged 18 February 1800 to Haslar.

Folios 7-10: John Collins, aged 30, landsman; disease or hurt, 3 foul ulcers about the size of a shilling on the fore part of the right leg, the discharge thin and reddish integuements of a livid appearance and much swelled. Put on sick list, 17 February 1800 at Spithead. Discharged 24 February 1800 to duty.

Folios 7-10: Charles M[Glanchy], aged 20, seaman; disease or hurt, gonnorhoea. Put on sick list, 17 February 1800 at Spithead. Discharged [?].

Folios 7-10: John Armstrong, aged 27, seaman; disease or hurt, venereal ulcer on the prepuce has been ill some days. Put on sick list, 17 February 1800 at Spithead. Discharged [?].

Folios 7-11: Samuel Turner, aged 23, marine; disease or hurt, chancres on the glans penis. Put on sick list, 17 February 1800 at Spithead. Discharged 27 February 1800 to duty.

Folios 7-21: Sergeant Bean, aged 27; disease or hurt, chancres slight swelling in left groin. Put on sick list, 17 February 1800 at Spithead. [Discharged 16 March 1800 to duty?].

Folios 7-23: Henry Turnball, aged 26, seaman; disease or hurt, paraphymosis chancres copious discharged from beneath the prepuce appears to be of general good habit but has been ailing some time without complaining, there are chancres on the glans penis. Put on sick list, 17 February 1800 at Spithead. Discharged 19 March 1800 to duty.

Folios 7-28: John Staples, aged 24, seaman; disease or hurt, bubo right groin but not inflamed. Put on sick list, 17 February 1800 at Spithead. Discharged 26 March 1800 to duty.

Folio 8: Samuel Burgess, aged 25, marine; disease or hurt, gonorrhoea and costive belly. Put on sick list, 21 February 1800 at Spithead. Discharged [?].
