Folios 3-4: case no 1, James McLean, aged 29, convict; taken ill at Sheerness; sick or hurt, phthisis; put on sick list 21 May 1833, died 14 June 1833 about 8 am.
Folio 5: case no 2, Thomas Allen, aged 25, convict; taken ill at Sheerness; sick or hurt, catarrhus contagiosus; put on sick list 3 May 1833, discharged 31 May 1833 cured.
Folio 6: case no 3, James Cannon, aged 28, convict; taken ill at Sheerness; sick or hurt, rheumatism; put on sick list 3 May 1833, discharged 10 June 1833 convalescent.
Folio 7: case no 4, James Heckingbottom, aged 19, convict; taken ill at sea; sick or hurt, variola discreta; put on sick list 19 May 1833, discharged 4 June 1833 convalescent.
Folios 7-8: case no 5, Robert Skewington, aged 26, convict; taken ill at sea; sick or hurt, ulcus; put on sick list 26 May 1833, discharged 20 August 1833 cured.
Folios 8-9: case no 6, William Walker, aged 25, convict; taken ill at sea; sick or hurt, phthisis; put on sick list 30 May 1833, died 16 June 1833 at 5 pm.
Folio 9: the following case no 7-9, were attacked about the same period with the usual symptoms of small pox (Cabin and Millard) got through the different stages of the disease very easily, the symptoms were mild throughout, they required little attention, and they were placed upon hospital diet. (Atkins) was equally easy in the disease and he suffered only from the profuse crop, which kept him longer upon the convalescent list.

Folio 9: case no [9], Charles Atkins, aged 16, convict; taken ill at sea; sick or hurt, variola discreta; put on sick list 9 June 1833, discharged 26 July 1833.
Folios 10-12: case no 10, Robert Watts, aged 68, convict; taken ill at sea; sick or hurt, ophthalmia, having received a thrust in the right eye from some obtuse instrument, but could not discern the person who inflicted the violence from having lost the sight of the left eye for some years; put on sick list 23 June 1833, died 3 September 1833.
Folio 11: case no 11, Daniel Bincks, aged 22, convict; taken ill at sea; sick or hurt, variola discreta; put on sick list 24 June 1833, discharged 20 July 1833.
Folio 12: case no 12, Thomas Carthey, aged 39, soldier; taken ill at sea; sick or hurt, asthma; put on sick list 3 July 1833, discharged 19 July 1833.
Folios 13-14: case no 13, William Townsend, aged 26, convict; taken ill at sea; sick or hurt, typhus meteor; put on sick list 28 July 1833, discharged 30 August 1833.
Folios 14-15: case no 14, James Hill, aged 27, convict; taken ill at sea; sick or hurt, scorbutus; put on sick list 18 August 1833, died 20 September 1833 about 8pm.
Folios 15-16: case no 15, Jonathan Gittins, aged 68, convict; taken ill at sea; sick or hurt, abscessus; put on sick list 21 August 1833, discharged 21 September 1833 well.
Folio 16: nosological return of cases mentioned in the journal.
Folios 17-18: Surgeon general's remarks. The total number of convicts embarked were 260 from Woolwich, Chatham and Sheerness. Four deaths occurred during the voyage to Sydney. Scorbutus was prevalent on board towards the end of the voyage, those advanced in years and others who had been subject to irregularities in life, suffered in a greater degree, also cases of small pox appeared on board immediately after quitting the Channel.
