Folios 1-2: case no 1, George Hamilton McKay, aged 24, convict, taken ill in the Downs; sick or hurt, dysenteria; put on sick list 9 March 1830, discharged 16 March 1830.
Folios 2-5: case no 2, Mrs Heward, taken ill [in the Downs]; sick or hurt, febris puerperalis; put on sick list 16 March 1830, discharged 5 April 1830.
Folios 4-5: case no 3, James Turner, aged 32, convict, taken ill at Mother Bank; sick or hurt, dysenteria; put on sick list 19 March 1830, discharged 27 March 1830.
Folios 5-8: case no 4, Richard Papps, aged 36, convict, taken ill at Plymouth; sick or hurt, hydrothorax, difficulty of breathing which in the recumbent posture become worse and accompanied with wheezing, frequent fits of coughing by which he throws out much mucus, bowels habitually slow, on 17th April symptoms not at all relieved has passed last 24 hrs about 2 ½ litres of urine lighter in colour, but on standing deposits more copiously a lateritious sediment, at 11 pm found his difficulty of breathing increased to such a degree that it could be performed only in sitting posture, frequent cough accompanied with a harsh sibilant sound and expectoration of tenacious mucus, the action of the heart very irregular and apparently the diastole performed with more difficulty, the inferior extremities seems cold and lifeless; put on sick list 5 April 1830, died 17 April 1830 at 4 am.
