Folios 7-9: case no 2, William Felder, aged 22, private 80th regt.; taken ill at Falmouth; sick or hurt, fever; put on sick list 12 June 1837, discharged 24 August 1837 cured.

Folios 9-12: case no 3, Adam Bailly, aged 24, private 80th regt.; taken ill at sea; sick or hurt, concussion and fever; put on sick list 14 June 1837, died 17 June 1837 at 11 pm.

Folios 12-15: case no 4, Richard Edwards, aged 20, convict sailer; taken ill at [sea?]; sick or hurt, synochus, this young man who was master of a Falmouth Schooner trading to the [?gores] was tried together with his mate for [insulting?] towards a sailer boy who died in consequence, they were found guilty of manslaughter and sentenced to the transported for life, he is apparently of a mild and harmless and well educated, has suffered great moral depression; put on sick list 28 June 1837, died 6 July 1837.

Folios 27-28: Surgeon's general remarks. On 1 June 1837, 250 convicts were embarked at Portsmouth on board all apparently in good health. The major part of the convicts were from London prisons. At the time of embarkation there prevailed on board the convict hulks a strong scorbutic diathesis and was under the necessity of rejecting a number, in whom such symptoms were present. Makes mention of case no 1 [Thomas Boyles], case no 3 [Adam Bailly] and case no 4 [Richard Edwards].
