Folio 1: Title page giving date range to 22 July 1799.
Folio 2: John Fletcher, Seaman, aged 46; disease or hurt, violent griping pains in his bowels, sickness at the stomach with frequent stools. Taken ill, 15 August 1798, at anchor off Cadiz. Discharged to duty, 19 August 1798. Remarks that many of the crew had similar symptoms, all responded to the same treatment.
Folio 2: Patrick Murphy, Seaman, aged 25; disease or hurt, same symptoms as John Fletcher. Taken ill, [...] August 1798. Discharged to duty 20 August 1798.
Folio 2: James Walsh, Seaman, aged 22; disease or hurt, violent pains across his loins about the epigastric region, from a blow from a rope, some difficulty breathing. Taken ill, 21 August 1798, at anchor off Cadiz. Treatment concluded on folio 3. Discharged to Gibraltar Hospital, 24 November 1798.
Folio 3: Orlando Bagley, Seaman, aged 33; disease or hurt, extremely debilitated, violent cough, difficulty breathing and loss of appetite. Taken ill, 22 August 1798, at anchor off Cadiz. Discharged to Excellent for passage to England, 3 October 1798. Surveyed by two surgeons of the fleet, 1 October 1798, sent to hospital for change of air and diet.
Folio 4: James Love, Marine, aged 26; disease or hurt, pain in his head and back, sickness at stomach. Taken ill, 24 August 1798, at anchor off Cadiz. Treatment continues folios 8 and 14. Discharged to Gibraltar Hospital, 8 October 1798. Undated note 'Since died'.
Folio 5: John [James] Skimmington, Seaman, aged 25; disease or hurt, pain in his head and left side, difficulty breathing with some cough, pulse regular, has every appearance of being unhealthy, countenance sallow, often complaining. Taken ill, 31 August 1798, at anchor off Cadiz. Treatment continues folios 9 and 13. Discharged to Gibraltar Hospital, 8 October 1798.
Folio 6: George Frost, Marine, aged 28; disease or hurt, pain in all his limbs, headache, great thirst, sickness at stomach with purging and pains in [his] bowels. Taken ill, 2 September 1798, at anchor off Cadiz. Discharged to 9 September 1798.

Folio 7: James Randle, Seaman, aged 26; disease or hurt, large round blotches all over his body, some converted in to sores, pains in his limbs at night, feels himself getting weaker. Taken ill, 2 September 1798, at anchor off Cadiz. Discharged to duty, 24 September 1798.
Folio 7: Nicholas Honbury, Seaman, aged 22; disease or hurt, glans much enlarged with a phimosis. Taken ill, 3 September 1798, at anchor off Cadiz. Discharged to duty, 14 October 1798. Was sent on board Edgar 3 September 1798, had been ill for several weeks.

Folio 16: James Cavanah, Seaman, aged 22; disease or hurt, violent pain in his breast, very thirsty, no stool for several days, skin dry and hot. Taken ill, 8 October 1798, at anchor off Cadiz. Treatment continues folios 18, 21 and 29. Discharged to Gibraltar Hospital, 24 November 1798.

Folio 19: Peter McCarthy, Marine, aged 25; disease or hurt, lacerated forehead with part of the bone exposed, by a blow from a block. Taken ill, 20 October 1798, off Cadiz. Discharged to duty, 15 November 1798.
Folio 20: William Lawrence, Seaman, aged 37; disease or hurt, stunned by a blow from a falling block, slight haemorrhage from the nose. Taken ill, 20 October 1798, cruising off Cadiz. Discharged to duty, 1 November 1798.
Folio 20: John Tomlin, Marine, aged 24; disease or hurt, inflammation and swelling of the testicle and spermatic [process] from a violent blow. Taken ill, 24 October 1798, cruising off Cadiz. Treatment continues folios 25, 31 and 42. Discharged to duty, 11 February 1799.
Folio 21: Thomas Dalton, Marine, aged 19; disease or hurt, a violent blow across the loins, causing great pain but with no visible injury. Taken ill, 26 October 1798, cruising off Cadiz. Discharged to duty, 7 November 1798.
Folio 21: James Cavanah, treatment continued.
Folio 22: William James, Seaman, aged 26; disease or hurt, struck by lightning while aloft. Taken ill, 29 October 1798, cruising off Cadiz. Died 1 November 1798. The accident happened at about 10pm. Clothes burned, his body scorched and to all appearances dead, no external injury apart from scorching. Continued two days in a 'stupid state', complained that 'all his misery' was in his belly which was very tense and inflamed.
