Folio 1: Blank.

Folio 2: Jeremiah Croneen, aged 36, able seaman, taken ill at Port Royal; sick or hurt, by the sentence of a court martial went through the fleet to received 500 lashes but after receiving 275 the sun becoming strong and he faint the cuts was completely removed from both shoulders and the upper part of his back; taken ill on 4 January 1806, discharged 10 January 1806 to duty.

Folio 2: James Miller, aged 41, gunner's crew, taken ill at Port Royal; sick or hurt, contusion on the right knee by a fall from the boom on the main deck; taken ill on 9 January 1806, discharged 14 January 1806 to duty.

Folios 2-3: John Cloake, aged 26, carpenter's crew, taken ill at Port Royal; sick or hurt, chissell wound across the palm of his left hand; taken ill on 11 January 1806, discharged 19 January 1806 to duty.

Folio 3: John Brown, aged 24, ordinary seaman, taken ill at Port Royal; sick or hurt, pain in his head back and extremities attended with severe fits; taken ill on 14 January 1806, discharged 21 January 1806 to duty.
