Folio 15: William Simpson, aged 26, seaman, taken ill at sea; sick or hurt, venereal a large chancres on the glans penis; taken ill on 10 November 1815, discharged 8 December 1815 to duty.

Folio 15: Thomas Smith, aged 43, marine, taken ill at sea; sick or hurt, rheumatism; taken ill on 17 November 1815, discharged 30 November 1815 to duty.

Folio 15: George Baker, aged 35, seaman, taken ill at sea; sick or hurt, catarrh; taken ill on 29 November 1815, discharged 28 December 1815 to duty.

Folio 17: Mr Davison, master, taken ill at sea; sick or hurt, fell down on deck in consequence of the ship rolling very much received a contused and lacerated wound over the os.occipitis; taken ill on 16 December 1815, discharged 22 December 1815 to duty.

Folio 18: [John] White, aged 35, seaman, taken ill at Spithead; sick or hurt, accident fell from the main rigging and struck the chains and went into the water; taken ill on 23 December 1815, discharged 4 January 1816 to duty.

Folio 18: 26 December 1815; Portsmouth Harbour, last night we had a great many accidents originating from drunkeness, the greater part are so slight as not to require them to be exempt from duty the remaining ones are not of any serious nature.

Folio 18: [John] Brown, aged 29, seaman, taken ill at Portsmouth; sick or hurt, wound brought into the bay in a state of intoxication has got a large lacerated wound over the upper of the right parietal; taken ill on 25 December 1815, discharged 4 January 1816 to duty.

Folio 18: James Mangold, aged 42, seaman, taken ill at Portsmouth; sick or hurt, wound brought into the bay in an insensible state fell out of his hammock and struck the back part of his head against one of the shaft lockers; taken ill on 31 December 1815, discharged 14 January 1816 to duty.

Folio 18: Luck [Luke] Hopkins, aged 31, marine, taken ill at Portsmouth; sick or hurt, accident in turning into his hammock fell and struck the gun with his right shoulder; taken ill on 31 December 1815, discharged 19 January 1816 to duty.

Folio 19: John Weston, aged 24, seaman, taken ill at Portsmouth; sick or hurt, has been in the list for catarrh previously, complaints of a sense of suffocation difficulty of breathing increased until 9:30 when he died at this time a quantity of florid blood came from his mouth and nostrils; taken ill on 4 January 1816, died 8 January 1816.

Folio 19: John Leyland, aged 41, marine, taken ill at Portsmouth; sick or hurt, chronic hepatitis he was sent on board from Malta hospital for a passage to England at present appears to be acute rheumatism; taken ill on 4 January 1816, discharged 9 January 1816 to hospital at Haslar.

Folio 19: William Oxford, aged 18, marine, taken ill at Portsmouth; sick or hurt, cough pain in the breast and difficulty of breathing; taken ill on 11 January 1816, discharged 13 January 1816 to hospital at Portsmouth.

Folio 19: William Hill, aged 35, seaman, taken ill at Portsmouth; sick or hurt, fell from the gangway between the ship and hulk he appeared half drunk; taken ill on 12 January 1816, discharged 13 January 1816 to hospital at Portsmouth.

Folios 19-20: Henry Brown, aged 23, marine, taken ill at Portsmouth; sick or hurt, brought on board the hulk from the Rivoli where he had been sentry got drunk and fell down one of the mast holes by which he received a wound over the right parietal; taken ill on 17 January 1816, discharged 18 January 1816 to hospital at Portsmouth.

Folio 20: [John] Walker, marine, taken ill at Portsmouth; sick or hurt, accident fell down the after hatchway into the spirit room of the hulk received a concussion; taken ill on 18 January 1816, discharged 19 January 1816 to hospital at Portsmouth.

Folio 20: 19 January 1816; Portsmouth, all the seaman were paid off the officers and marines remain, we have two slight accidents on the list at present, I have previously sent to the hospital those who were not in a fit state to go ashore.

Folio 20: 21 January 1816; Mr Malcolm assistant surgeon who has been in the prize frigate Melpomene from the time of our leaving Malta applied to me today with large chancres on the prepuce as we are all in an uncertain state on board I thought it necessary for his cure to send him to the hospital on 21 January 1816 at Portsmouth.

Folios 20-21: numerical abstract of cases listed in the journal and general remarks on surgeon's complaint that in 1816 the sick berth was converted into a prison for smugglers.
