Folio 2: Mr Bowen, aged 28, Surgeon; disease or hurt, was attacked with general lassitude and nausea accompanied with headache, pain in the back and lower extremities; taken ill, 12 October 1804 during passage from Surinam to Barbados, died 14 October 1804.

Folio 3: George Wilson, aged 36, Master, disease or hurt, attacked with acute head pain and pain in the back extending to the loins and lower extremities; taken ill 12 October 1804, died 15 October 1804.

Folio 3: Daniel Hale, aged 22, Marine; disease or hurt, attacked with acute headache, pain in back extending to the loins and lower extremities and fever; Taken ill, 12 October 1804 during passage from Surinam to Barbados; discharged to hospital in Barbados, 16 October 1804.

Folio 7: William Hooper, aged 24, marine; William Webb, aged 37, Seaman; Jonathan Foster, aged 29, Cook; Jonathan Scott, aged 20; Jonathan Beazley, aged 32; Joseph Dyer, aged 25; were all attacked with fever 23 October 1804 and sent to the hospital the same day.
