Folios 1-4: case no 1: Joseph Lawrence, aged 25, seaman, native of St Thomas's; disease or hurt, catarrhus with symptoms of pneumonia, gastric and hepatic derangement; taken ill at Plymouth Sound, 18 March 1827; discharged 8 May 1827 as a convalescent to the Royal Naval hospital, Bermuda.
Folio 4: case no 2: John Brown, aged 41, ship's cook, on passage from England to America; disease or hurt, enteritis; taken ill at sea, 23 March 1827; discharged 2 April 1827 to duty.
Folios 5-6: case no 3: John Barry, aged 21, seaman; disease or hurt, phlegmonsas erythema; taken ill at Bermuda, 9 May 1827; discharged to duty first on 2 June 1827, after relapse, 11 June 1827.
Folios 6-8: case no 4: Henry Thomas, aged 35, seaman; disease or hurt, pneumonia; taken ill at Halifax harbour, 1 June 1827; discharged to Halifax hospital on 7 June 1827 in almost a state of convalescence.

Folios 10-13: case no 6: John Harrier, aged 22, captain's servant; disease or hurt, rubella vulgaris; taken ill at the river of St Lawrence, passage from Quebec to St John Newfoundland, 10 July 1827; discharged to duty on 31 July 1827.
Folios 14-15: case no 7: Mr Lark, aged 26, second master; disease or hurt, rheumatismus aentus; taken ill at Halifax harbour, 25 May 1827; discharged to duty on 5 June 1827. Suffered a second attack of rheumatism on 22 August 1827 sent to sick quarters in a state approaching convalescency on 29 August 1827.

(Names and details follow): Thomas Barker; age, 41, rating, boatswain's mate; ship pay book number, 44; pension certificate granted; date of certificate, 31 August 1827; hurts [wounds] received: fell, on the 9 May 1827, then engaged in the discharge of his duty, from the main to the lower deck, and was in consequence, ruptured in the left groin. Rupture an Oscheocole and reducible then the certificate was granted. John Brown; age, 41, rating, ship's cook; ship pay book number, 4; pension certificate not granted; date of certificate, 31 August 1827; hurts [wounds] received: on the 23 June 1827, fell, during a gale of wind from the main to the lower deck and thereby had his skull fractured in the right parietal bone, bone slightly depressed. George Martin; age, 37, rating, captain of the forecastle; ship pay book number, 111; pension certificate not granted; date of certificate, 31 August 1827; hurts [wounds] received: on the 2 August 1827, fell from the foreyard to the forecastle, and thereby had his right thigh bone fractured, right humerus dislocated, right internal [plin?] artery torn, the right orbital plate fractured and depressed, right eye brow lacerated. John Carter; age, 19, rating, ordinary seaman; ship pay book number, 76; pension certificate not granted; date of certificate, 31 August 1827; hurts [wounds] received: received a severe contusion of right ankle by a pig of ballast falling upon it, then he was performing his duty.
