Folio 1: case no.1, John Greme, aged 50, able seaman; taken ill at West Indies; disease or hurt, previously invalided for rheumatism, much debilitated; taken ill, 24 June 1810; sent 11 September 1810 to hospital at Plymouth.

Folio 2: case no.2, Elias Brumere, aged 40, able seaman; taken ill in Channel; disease or hurt, feverish with extensive inflammation in the left leg with severe pain; taken ill, 4 August 1810; discharged 21 September 1810 to duty.

Folio 2: case no.3, Robert Madget, aged 38, able seaman; taken ill at Hamoaze; disease or hurt, ulcer on the left leg; taken ill, 30 August 1810; discharged 26 September 1810 to duty.

Folios 2-3: case no.4, John Hard, aged 19, ordinary seaman; taken ill at Hamoaze; disease or hurt, eruption over the whole body particularly spread over superior and lower extremities; taken ill, 30 August 1810; discharged 1 November 1810 to duty.

Folio 3: case no.5, John Maxwell, aged 56, ordinary seaman; taken ill at Hamoaze; disease or hurt, rheumatism previously in the list on 4 August last for catarrh; taken ill, 1 September 1810; invalided out of the service, 10 September 1810.

Folio 9: case no.17, John Muan, aged 23, landsman; taken ill in the Western Ocean; disease or hurt, phlegmon on the left ankle and subject to scorbutic ulcers on the legs; taken ill, 3 October 1810; sent 15 December 1810 to hospital in England as a result of a survey.

Folios 10-11: case no.18, Thomas Carter, aged 26, able seaman; taken ill at Hamoaze; disease or hurt, hemorhoidal[hemorrhoid] tumors; taken ill, 12 September 1810; died 23 February 1811.
