Ref: 4366SR/5. The failure was analysed as follows: 1 Fatigue failure of four 1st stage blades; two blades had fractured through the top root serration; one blade was cracked in the top serration and one cracked in the bottom serration. One fractured blade had fatigued over about one-third of the area of the top serration. 2 Fatigue failures of three 2nd stage blades, one blade had fractured through the top root serration and was also cracked in the bottom serration
